HYGEIA BILLING POLICIES AND FEES
As of 1 January 2009, Hygeia will contract only with Anthem BC/BS. Any patient with BC/BS insurance will have their visits billed for them with payment due according their insurance coverage.
Patients with private insurance other than BC/BS will be expected to pay for their visits in full at the time of service. Payment can be made by cash, check or credit card. You will receive a coded invoice from Hygeia which you can submit to your insurer for reimbursement according to your policy.
Medicare patients will not be eligible for reimbursement for their visits with Dr. Jacobs.
Payment plans are available upon request but must be arranged prior to your visit. Please contact the office to set up an appointment to discuss this option.
