HYGEIA FEE SCHEDULE
The fees below reflect careful consideration of the unique nature of our medical approach, taking into account the amount of review and follow-up work that takes place before and after appointments in addition to the face to face time you spend with Doctor Jacobs. We have worked hard to create a fee schedule which allows us to spend the time we need to provide you with exceptional care while simultaneously honoring and respecting the difficult economic issues so many of us are experiencing.

| Functional Intake Evaluation (80 min MD visit, 60 min nutrition/lifestyle assessment) Includes: Review of Records and any available medical notes or tests prior to your first meeting with Dr. Jacobs, Comprehensive History, Review of Medical Symptom Questionnaire, Body Composition Analysis, Physical Exam and Post-visit Review of your case and any additional medical information brought to Dr. Jacobs' attention at the time of the evaluation. Recommendations for testing will be made. Approximately 7-10 days later you will meet with Holly Westling, RN, MS for a dietary/lifestyle assessment and to begin implementation of therapeutic lifestyle elements of your treatment plan. |

Ongoing support provided in 5-7 individual lifestyle coaching sessions with Holly over 12 weeks. Each coaching session will include Body Composition Analysis, assessment of progress, and program modification based upon personal health status goals with review of all records by Dr. Jacobs. Nine months of complementary enrollment in our FlightTeam immediately following completion of the program. Cancellation and No Show Policy
Last minute cancellations and no shows are disruptive to our schedule and prevent other patients from being able to obtain care in a timely manner. Please make every effort to keep your appointment as scheduled. If you must reschedule please notify us as soon as possible so that we can make your appointment time available to someone else. Patients cancelling with less than 24 hours notice will be charged 50% of the fee for their scheduled appointment, patients who miss an appointment without notification will be charged in full for the appointment.
Insurance Billing
Hygeia is currently contracted with Anthem Blue Cross and Blue Shield of NH. Patients with a BC/BS insurance plan will have the charges for standard medical services submitted to insurance. Functional and Lifestyle medicine approaches offered at Hygeia are not reimbursable through the standard medical model. New patients will have $270 of the initial visit billed through insurance. The remaining $180 (for the Functional assessment and nutrition/lifestyle evaluation) will be collected at the time of service.
Patients who do not have insurance or have insurance other than BC/BS will be expected to pay in full at the time of service. Patients with non-BC/BS insurance who would like to request reimbursement for services will be given a coded receipt to submit to their insurers.
Medical coverage is plan-dependent and we encourage all of our patients to contact their insurer and educate themselves about their policy. Dr. Jacobs is not contracted with Medicare and patients are therefore not eligible for reimbursement from Medicare for any services provided by Hygeia.
